TRAVEL SCHOLARSHIP APPLICATION
Our nonprofit organization has the ability to provide travel assistance to healthcare professionals to attend, participate in a Judith C Crosby Neuroscience Foundation-related conference that would increase the neuroscience knowledge of the attendee.
ELIGIBILITY
Only requests which meet the following criteria will be considered for a travel scholarship:

Program relevant to current role

Travel request limited to coach airfare/train/mileage and moderate hotel (room and taxes only)

Applicant must attend entire conference

Application approved prior to conference and expenses
APPLICATION PROCEDURE
The application must be complete and submitted 21 days prior to the conference. Any application received after mid-August 2019 may be considered for the next scheduled session. Members of the JCCNF Board of Directors will approve travel scholarships and notify recipients.
TERMS OF AWARD
If participant is unable to attend, a designee maybe submitted, but applicant may be responsible for change or cancellation fees. Itemized original expense receipts must be submitted immediately following the conference. NO MEALS or ALCOHOL expenses will be reimbursed. JCCNF may choose to prepay expenses if warranted.
